A “down payment” of $500 and monthly payments of $110.00 entitles you to the following benefits each calendar year:

Additional Membership Rules:
- Additional units may be purchased during your visit at a discounted per unit rate.
- A minimum 12-month commitment required. If you cancel your membership before the end of the 12-month membership, you will be required to pay for any BOTOX® utilized but not yet paid for at the regular unit rate.
- A 30-day written notice is required for all cancellations. Please contact our office for assistance.
- Membership discounts are not combinable with other offers or discounts. This membership does not renew and annual re-enrollment is required.
